Questions & Answers (Forms)
Wootzoo calls its forms Questions & Answers (Q&A). If you're looking for a "create a form" button, this is where it lives.
Where to Find It
In the left-hand menu, under your setting's name, click Questions & Answers. Then click Create New to open the form builder.
What You Can Build
- Registration forms for new families
- Allergy and dietary questions
- Permission slips and trip consents
- Any other questions you need to ask
Where Answers Go
Answers are saved against the child's profile, so you can view them any time from the child's page.
If you tick "show on register" on a question - allergies, for example - the answer appears as a small badge next to the child's name on the SerenPass register. That means staff can check needs at sign-in without navigating away.
Sending a Form to Parents
From the Q&A list, choose a form and then send to parent. Forms can also be set as required at registration, so new families must complete them before their registration is accepted.
Your Forms and Built-in Forms
The Q&A list is split in two. Your Forms shows the forms you have created yourself. Built-in forms shows the standard Wootzoo forms, such as the medication form and the Child Collection Form. Each form shows whether it is Active or Inactive, with active ones first. Click a form to open it and change its settings.
Turning a Form Off
If you've made your own version of one of the standard forms (a medication form, for example), you can turn the standard one off so parents don't fill it in by mistake.
- Click Questions & Answers and open the form you want to turn off. The standard forms are under Built-in forms.
- In the Form Settings box on the right, untick Active.
- Click Save Settings.
Parents will no longer see the form, and any answers already submitted are kept. You can turn it back on at any time. Any links you've already shared stop working while it's off.
