Forms & Medication

Your setting can set up forms that you send to parents to fill in - for example a medication form for ongoing medication. Here's how to find and send one.

Sending a Form to a Parent

1. Go to the child's profile page.
2. Scroll down to the Send Form to Parent card.
3. Pick the form (e.g. Medication Form) from the list, and pick the parent.
4. Tap Send Form Request. The parent is notified and fills it in on their app.

Medication Forms

The Medication Form covers long-term/ongoing medication as well as one-off medication. Every past submission is listed on the child's profile as a medication history.
